OneCare allows you to specify your health and social care needs. You can specify who will provide the care, what type of care you need, and how it should be given. The care plan is important because it ensures that you get the appropriate care needed regardless of which care worker is on duty at a time. In the care plan, certain things can be specified, like the assessed care needs tailored to you, what support you should receive, your desired outcomes, who should provide the care, when the care should be provided, the costs of services, and your wishes and personal preferences.
